What is IVF ?

Do you remember Louise Brown, born in England, in 1978? Yes, she was the first test tube baby. She was conceived outside her mother's womb. During those days it was considered something mysterious, but now, IVF is a household name. In vitro means outside the body. IVF is a form of Assisted Reproductive Technology (ART). It is a treatment for infertility as a result of which you can conceive a child even after other medical interventions have failed.

When to consider IVF

IVF is an option for women above 40, because both egg quantity and quality decline with aging.

If you or your partner is suffering from the following ailments, you must go for IVF treatment:

  • Damage or blockage of fallopian tube
  • Ovulation disorders - Patients with disorders like PCOS, hypothalamic amenorrhoea or ovarian failure may require IVF.
  • Endometriosis - It affects the functioning of ovaries, uterus, and fallopian tubes.
  • Uterine fibroids - Benign tumours in the uterus, called fibroids can interfere with the implantation of fertilized eggs.
  • Multiple miscarriages - In such cases,IVF treatment is a hope.
  • Unsuccessful fertility treatments - When non-invasive treatments like fertility medicines and IUI have failed, IVF can be chosen.
  • Previous tubal sterilization, or tubal ligation.
  • Impaired sperm production, abnormal size, shape, or function.
  • Fertility Preservation for Cancer or other health conditions.

Kinds of IVF

IVF can be between

  • IVF + Frozen embryo transfer (FET)
  • IVF + Elective Single Embryo Transfer (eSET)
  • IVF + Intracytoplasmic Sperm Injection (ICSI)
  • IVF + Egg donor
  • IVF + Donor sperm
  • Natural IVF
  • Mini IVF (minimal stimulation)

Workflow

The IVF procedure involves the following steps:

01
Ovarian Stimulation

Starting from the second day, regular injections are given for follicle growth. Ultrasound scans help in monitoring the growth and also in evaluating the uterine lining. The combination of medications and injections provide stimulation to the ovary. Blood tests are also performed to examine the level of hormones.

02
Administration of Trigger

When the follicles attain an optimum size of 18-20 mm, an injection called trigger is administered for the maturation of the egg.

03
Egg retrieval

Egg retrieval is carried out 34-36 hours after the administration of the trigger. Multiple eggs can be retrieved in just 20 minutes

04
Sperm retrieval

On the day of egg retrieval, the partner's sperm is collected from their semen sample, in the lab

05
Insemination

The semen sample and the eggs are mixed together in a petri dish.

06
Fertilization

The retrieved eggs can be fertilized with the sperm either by overnight incubation or injection of the healthy sperm directly into the egg.

07
Cultivation of Embryo

Depending on the quality, the embryo is cultivated for up to five days in the laboratory.

08
Transfer of Embryo

Embryo is transferred into the uterus 2-5 days after the egg retrieval. On the fifth day, the embryo is a blastocyst and when blastocyst is transferred, pregnancy chances are improved.

09
Blood test for pregnancy

A blood test is performed for pregnancy two weeks after the date of embryo transfer.

FAQ

Serious complications are rare during IVF. The common risks are multiple pregnancy, premature delivery, Ovarian Hyperstimulation Syndrome, miscarriage, bleeding, infection, damage to organs near the ovaries, ectopic pregnancy i.e egg implantation outside the uterus, and stress.

Due to cycle-to-cycle variability in egg quality and since ovarian stimulation can be modified based on what was learned from the previous IVF attempt/ attempts, in most cases, IVF is worth trying again.
